Brownfields Program - Forms
The Brownfields Program is developing a complete record of sites that may be contaminated, called the "Inventory". If you live in a Consortium community, and would like to report a site, click on the link below.
Formal Notice of Potential Brownfield Site Community of Nelson Island Consortium
Fill out the form and fax it to the Brownfield Response Program in Tununak at (907) 652-6511. Or you may bring it to your local Consortium Brownfield Representative at your Tribe's Environmental Program Office. You may also call or stop by the Office to fill out this form with our staff (see Program Contacts). For any questions on the form, please contact your local Brownfield Representative, or Joey Billy at (907) 652-6537.
Once the form is received by the Brownfield Program, the reported site will be evaluated in greater detail by your local Brownfield Representative. They will determine whether action is needed immediately and whether the site should be included in the formal Inventory. If you do not have a Representative in your Village, a Representative from another Village will be assigned to work on the form. A Public Record is a list of inventoried sites for which action has been taken or will be taken in the next year. If you are interested in a site property, you may view the Nelson Island Consortium Record to see whether any restrictions have been placed on its use due to contamination concerns. The Record is also available on the designated Brownfield computer in each Environmental Office.
